Where the applicant requested that a certificate of title was not to issue on registration of a dealing, e.g. a resumption of land or road closing, the applicant may apply for issue of the certificate of title at a later date by lodging a Request form 11R. The applicant must be identical to the registered proprietor as shown on the Torrens Title Register.

Note  A letter received at a later date requesting issue of the certificate of title is not acceptable.

(B) The reference to title for the land affected by the request must be stated.

(C) Does not apply.

(E) The full name of the registered proprietor must be stated and be identical to the name as shown on the Torrens Title Register.

(F) A short description of the nature of the request must be stated.

(G) Full details of the request must be stated.

The registered number of the relevant dealing must be stated.

(H) The dealing must be executed by all of the registered proprietors and be witnessed, or be executed on the registered proprietors behalf as follows:
